ABSTRACT

Objective:To explore the effect of MDR1(C3435T) gene polymorphism on tacrolimus metabolism early after pediatric liver transplantation.Methods:Preoperative blood samples of 90 donors and recipients of pediatric liver transplantation were collected and genotyped. According to the CYP3A5 genotype of donor/recipient, they were divided into four subgroups of recipient slow metabolism/donor slow metabolism (R/D-S), recipient fast metabolism/donor slow metabolism (R-F/D-S), recipient slow metabolism/donor fast metabolism (R-S/D-F) and recipient fast metabolism/donor fast metabolism (R/D-F). The values of concentration/daily dose (C 0/D) of tacrolimus in patients with different MDR1 genotypes were compared at the subgroup level. Results:The C 0/D value of MDR1 TT recipients was significantly higher than that of CC/ CT counterparts in the first week after liver transplantation ( P<0.01). The C 0/D value of CT recipients in R/D-S subgroup was significantly higher than that of CC counterparts in the 2nd week after operation ( P<0.05). The C 0/D value of CT recipients in R/D-F subgroup was significantly higher than that of CC counterparts in 2 weeks ( P<0.05) and 3 weeks ( P<0.01). Conclusions:MDR1(C3435T) gene polymorphism in recipients affects tacrolimus metabolism. Recipients with CC genotype metabolize faster than those with CT/TT genotypes. And the same daily dose and tacrolimus blood concentration are lower. Recipients with different MDR1 genotypes need to adjust the dosage of tacrolimus. This difference is more obvious in CYP3A5 fast metabolic subgroup, more attention should be paid to optimizing individualizing dosage regimens, reducing the incidence of adverse reactions and improving the efficacy.

ABSTRACT

Objective:To develop a UPLC method for determining four glycosides in microctis folium.Methods:The UPLC deter-mination was performed on an Agilent SB -C18 (3.0 mm ×50 mm, 1.8 μm) colume with 0.05% acetonitrile -phosphate solution as the mobile phase.The detection wavelength was set at 320 nm.The flow rate was 0.3 ml· min-1.The column temperature was 25℃.Results:Narcissoside, isovitexin, kaempferol-3-rutinoside and isorhamnetin-3-O-glucoside all showed good linearity with the re-covery of 98.89%,99.03%, 97.00%and 97.41%, and RSD of 0.41%, 0.84%,0.44%and 0.80%, respectively (n=9).Con-clusion:The method is convenient with good stability , repeatability and sample recovery rate , which provides basis for the quality eval-uation and control of microctis folium.

ABSTRACT

Objective To investigate the clinical effect of different detection methods in the detection of allergens in allergic skin diseases .Methods Choose 74 cases of allergic skin disease patients in our hospital from March 2014 to March 2015 as the research object ,they were divided into two groups by random number method ,immunoblotting group and skin prick test group ,detected al-lergens of the two groups by immunoblotting and skin prick test before treatment .Results The positive rate was 70 .3% in immu-noblotting group ,and the positive rate was 56 .8% in skin prick test group .For allergens detection ,the positive rate was higher by using immunoblotting ,the differences between the two had significant difference(P<0 .05) .Conclusion Immunoblotting is better than the skin prick test for allergen detection in allergic skin diseases .

ABSTRACT

Objective: To develop an HPLC method for determining feruloyltyramine in Pothi chinensis herba. Methods: The HPLC determination was performed on a Thermo ODS-2 Hypersil column(250 mm × 4. 6 mm, 5 μm) with isocratic elution of metha-nol-0. 4% phosphoric (35:65) as the mobile phase. The flow rate was 1. 0 ml·min-1 . The column temperature was at 25℃, and the detection wavelength was set at 318 nm. Results: Feruloyltyramine showed good linearity,and the recovery was 99. 40% with RSD of 1. 44% (n =9). Conclusion: The method is quick, simple and reproducible, which can be used to control the quality of Pothi chinensis herba.

ABSTRACT

ObjectiveTo investigate the changes and significance of urine kidney injury molecule-1(KIM-1) in liver transplant recipients concurrent with early-stage acute kidney injury (AKI).MethodsThe clinical data of orthotopic liver transplantation in 50 cases was retrospectively analyzed.According to the Acute Kidney Injury Network (AKIN) criteria and whether there was AKI for recipients after operation,the recipients were divided into AKI group (27 cases) and non-AKI group (23 cases).Serum creatinine (SCr),urine creatinine (UCr) and urine KIM-1were determined at the scheduled time points,and relationship between urine KIM-1and AK1was analyzed.By using the receiver operating characteristic (ROC) and the area under the curve (AUC),the diagnostic accuracy of urine KIM-1for AKI was evaluated.ResultsThe level of SCr reached the highest in two groups at 24 h postoperation,that in AKI group was significantly higher than in non-AKI group (P<0.05),and then gradually decreased to the preoperative level.The level of urine KIM-1was significantly increased immediately at the time of portal vein reperfusion in two groups,and that in AKI group reached the peak at 2nd h after portal vein reperfusion,significantly higher than in non-AKI group (P<0.01),which continued 12 h after the portal vein reperfusion.The results showed that the sensitivity was 82.6% and the specificity was 88.9% when the urine KIM-114.19 ng/g Ucr was taken as the cutoff to the diagnosis of AKI two h after portal vein repeffusion.ConclusionThe level of urine KIM-1is a reliable biomarker to diagnose AKI after liver transplantation.The intraoperative changes of urine KIM-1may be helpful to early prediction of AKI,for recipients with preoperative normal renal function.

ABSTRACT

Objective To establish a method for determining the contents of chrysophanol, emodin and sodium Danshensu in Shen An Kang Capsules(SAKC). Methods Chrysophanol and emodin contents were determined on Lichrospher100 C18 column (4.6 mm? 250 mm, 5 ? m) with the mobile phase of Methanol- 0.1 % Perchloric acid (85 ∶ 15) at a flow rate of 1 mL? min-1, and the detection wavelength was 254 nm. Sodium Danshensu content was determined on Kromasil column C18( 4.6 mm? 250 mm, 5 ? m) with the mobile phase of actonenitirile - 1.2 % Glacial acetic acid(8 ∶ 92)at a flow rate of 0.5 mL? min-1, detection wavelength being 280 nm. Results The linearity of emodin was obtained in the range of 0.020 16~ 0.100 80 ? g(r=0.999 6), linearity of chrysophanol obtained in the range of 0.034 56~ 0.172 80 ? g(r=0.999 9)and linearity of sodium Danshensu obtained in the range of 0.144 8~ 0.868 8 ? g(r=0.999 6).The average recovery was 97.70 % with RSD 2.38 % for emodin, 97.69 % with RSD 1.92 % for chrysophanol and 97.52 % with RSD 0.77 % for sodium Danshensu. Conclusion The method is accurate, reproducible , and is helpful for the quality control of Shen An Kang Capsules.

ABSTRACT

Objective To establish a method for the Determination of Danshensu content in Tongmai Capsules. Methods The content of Danshensu was determined by HPLC on Kromasil C18 Column( 4.6? 250 mm, 5 ? m) . The mobile phase was acetonitri1- 1.2 % water solution of acetic acid (9∶ 91) and the detection wavelength was 280 nm. The theoretical plates should over 3000 according to Danshensu. Results Sodium Danshensu showed a good linearity in the range of 0.288~ 1.152 ? g, r=0.9994. The average recovery was 97.62 % , and RSD was 1.10 % (n=5). Conclusion This method is effective and can be used for the quality control of Danshensu in Tongmai Capsules.

ABSTRACT

AIM:To establish the quality control for Finger-citron Formula Granule. METHODS: The TLC,HPLC and fingerprint chromatogram were used to make quality standard. RESULTS: The TLC spots were clear and 5,7-dimethoxy-coumarin showed a good linearity at a range of 0.46-4.14 ?g.The average recovery was(99.01%)(n=6) with RSD 1.58%. CONCLUSION: The method could be used as the quality control of Finger-citron Formula Granule.
